病理生理学

因此,认知功能障碍是否与MDD的整体综合征相关,可能不如认知功能障碍是否能识别出MDD综合征中一个更具生物学同质性的亚群来得重要。

成人MDD的认知功能障碍

MDD的定义包括主观上抱怨思维或集中注意力的能力受损。MDD患者普遍会感到专注力下降和健忘。当对成年MDD患者进行正规认知测试时,发现其在显性言语和视觉记忆、注意力、言语学习、任务执行速度以及执行过程方面存在中等程度的异常。各项研究中最大的效应量出现在情景记忆的编码和提取领域,尽管这些异常的程度小于痴呆症、双相情感障碍或精神分裂症患者。17

关于MDD相关认知缺陷的文献非常广泛,但在许多关键点上有些不一致。虽然绝大多数研究发现了认知异常,但一些针对50岁以下成年人的研究仅发现轻度异常或认知功能正常化。18在一项大型欧洲研究中,对8229名较年轻(平均年龄48岁)的MDD门诊患者进行延迟回忆记忆测试,约2.5%被归类为正常或优于正常;15.5%被评为轻度受损,41%为轻度至中度受损,35%为中度至重度受损,仅6%为重度受损。4

认知缺陷通常在以下患者中更严重且更常见:

  • 年龄较大
  • 有更严重的MDD症状或精神病性/忧郁特征
  • 伴有纤维肌痛、心血管疾病或糖尿病
  • 具有与双相情感障碍风险增加相关的临床特征

近期研究表明,高皮质醇血症可能与年轻MDD成年人的记忆和执行功能受损有关。处理速度更依赖于抑郁症状的严重程度。19

许多重要问题仍未解决。关于认知症状是与特质相关还是与状态相关,仍存在一定程度的不确定性。20虽然许多研究发现,处于缓解期的MDD患者仍存在认知功能障碍,但许多治疗研究表明,大多数患者的认知指标有显著改善。问题还在于,动机减退、精神运动迟缓和注意力不集中对MDD患者所见的更广泛的认知异常有多大贡献。21

此外,虽然一些研究表明,病程越长或既往发作次数越多与更严重的认知功能障碍相关,但其他研究未能发现这些关联。

老年MDD的认知功能障碍

最可靠且一致性的认知异常已在老年MDD患者中被发现。5这也许并不奇怪,因为MDD患者所见的大部分认知损害与正常衰老过程中发现的认知损害非常相似,包括表现迟缓、注意力问题和短期记忆缺陷。22然而,使用具有相似程度心血管疾病风险因素的年龄匹配对照组的研究通常发现,MDD组有更严重的认知损害。4,23

在老年MDD组中,MRI扫描显示深部白质高信号体积更大的患者认知功能最差。10,24白质高信号在老年人群中很常见,在高达60%的健康老年人中都会出现。25然而,老年MDD患者通常比年龄匹配的对照者拥有更多深部白质高信号,并且这些病变的存在与更差的认知表现相关,尤其是在执行功能领域。15,26

近期研究表明,老年MDD患者的深部白质高信号位于与情绪调节和执行功能相关的脑区。更大的病变体积与执行功能损害相关。23老年MDD患者的海马体积减小也与更严重的认知功能障碍相关,特别是执行功能和记忆损害。

虽然一些证据表明海马体积减小与病程有关,但其他研究发现,50岁以后发病的MDD患者比早发MDD患者有更明显的海马体积减小。此外,高皮质醇血症和包含血清素转运体启动子基因某些等位基因的遗传背景都与海马体积减小相关。27

50岁以后发病且MRI扫描显示广泛深部白质高信号的MDD患者,可能代表了其疾病源于全身性血管疾病的一个独特MDD亚群。24血管性抑郁症亚群共病全身性疾病(如心血管疾病和糖尿病)的比例更高,并且该亚群患者更有可能发展为痴呆。10,28虽然许多人认为血管疾病可能在部分老年MDD患者中起因果作用,但对于定义血管性抑郁症的具体标准仍缺乏共识。近期的工作不再强调发病年龄,因为血管疾病显然也发生在年轻人和老年人中。Alexopoulos14强调执行功能障碍是核心特征,而Taylor及其同事29则侧重于MRI显示的血管病理证据。需要更多的工作来建立有效且可靠的血管性抑郁症标准。

此外,已知MDD易导致阿尔茨海默病,并且某些形式的MDD可能是阿尔茨海默病的前驱症状。30一项引人注目的近期研究发现,老年抑郁症患者血浆中β-淀粉样肽40(Ab40)与Ab42的比例高与认知功能障碍发生率高相关。由于Ab40与Ab42的比例与阿尔茨海默病风险相关,这些研究者提出,淀粉样蛋白相关的MDD可能是阿尔茨海默病的前兆,并且是MDD的一种独特形式。31需要后续研究来证实或否定这一假设。

总的来说,老年患者中中度至重度的认知功能障碍可分为以下三类:

认知功能障碍对MDD治疗的临床意义

大多数抗抑郁治疗研究发现,年轻成年人的认知功能随着治疗而改善,尽管处理速度可能随治疗反应改善得更明显,而记忆损害可能更可能持续存在。19在一项针对242名老年MDD患者的大型研究中,氟西汀和帕罗西汀改善了记忆力和注意力。32丙咪嗪和氟伏沙明也改善了注意力和处理速度的测量指标。33然而,在Nebes及其同事34对老年抑郁症患者进行的一项研究中,去甲替林和帕罗西汀均未改善认知能力:患者治疗后仍表现出显著的残留认知损害。

近期一项针对老年MDD患者的研究表明,艾司西酞普兰治疗与显著的记忆改善相关。35同样,一项关于血清素-去甲肾上腺素再摄取抑制剂度洛西汀的研究显示,记忆力和言语学习能力在治疗后显著改善。36

据报道,去甲肾上腺素能-多巴胺能抗抑郁药安非他酮也能改善一组年轻(平均年龄24岁)MDD成年人的多种认知症状。37安非他酮显著改善了视觉记忆和精神处理速度。治疗前视觉记忆低下和精神处理速度缓慢是安非他酮总体抗抑郁反应良好的预测因素。37

然而,多项研究发现,中度认知功能障碍预示着老年患者无法达到抗抑郁反应。38,39在一项研究中,对氟西汀无反应者表现出更多的持续性错误。38另一项研究表明,在一组接受西酞普兰治疗的老年MDD患者中,启动/持续能力的损害与反应不佳相关。15此外,研究发现氟西汀对伴有精神运动迟缓的MDD患者效果较差。40最后,一些研究者提出,通过使用一组认知功能测试,可以预测85%MDD患者的SSRI反应。41

结论

虽然许多研究注意到MDD患者存在显著的认知异常,但平均而言,这些障碍在年轻成年人中倾向于轻度至中度,其频率和严重程度低于双相情感障碍或精神分裂症患者。在年轻成年人中,一些数据表明,精神运动迟缓和动机及注意力下降可能导致了其他认知领域的明显功能障碍。年轻成年人的认知功能障碍往往随着治疗而改善,尽管一些患者仍存在残留的轻度损害。2虽然一项安非他酮研究表明,在基线时存在认知功能障碍的患者中,该药更有可能导致抗抑郁反应,但没有可用的比较数据。使用SSRIs的研究表明,基线认知损害、海马体积减小、白质高信号负担加重以及白质微结构异常可能预示着抗抑郁反应不佳,尤其是在老年患者中。42

根据我们目前所知,无论是否存在认知功能障碍,MDD患者都应接受标准的抗抑郁治疗。临床医生应警惕基线时认知功能障碍更严重的患者出现无反应,以及在抗抑郁治疗成功反应后仍有显著残留认知功能障碍的患者进展为明显的痴呆。

虽然一项研究中安非他酮在伴有认知功能障碍的年轻成年人中显示出更高的疗效这一事实很有趣,但未使用对照组,且这在老年患者中也未得到证实。因此,现有数据不支持在认知功能障碍患者中使用一种抗抑郁药优于另一种。如果痴呆症发生或与MDD同时存在,应采用抗抑郁药与批准用于治疗痴呆症的药物进行综合治疗。43
