背景:记忆和认知的进行性个体内部下降是痴呆的特征,可能有助于检测非常早期的阿尔茨海默病病理。
方法:本研究评估了263名50岁以上健康社区居住成年志愿者在12个月期间的认知表现斜率。参与者在基线以及3、6、9和12个月随访评估时完成了一套简短的计算机化认知测试(CogState)。使用线性混合模型来估计每个认知测量指标的年龄调整平均斜率和95%变化的置信区间。
结果:通过定义情景记忆测量的年龄调整平均斜率和95%置信区间,识别出认知下降大于预期(等于或低于下降的第五百分位水平)的个体。从这些个体中,四名完成了全面的医学、神经学和神经心理学评估,他们均不符合轻度认知障碍的标准,但有三名(75%)具有阳性的淀粉样蛋白-正电子发射断层扫描结果。
结论:在其他方面健康的社区居住老年人中可以检测到认知表现的个体内部下降,这可能值得进一步研究作为早期阿尔茨海默病病理的潜在指标。
